キャンプ参加同意書
① 保険について / Insurance
本キャンプでは、不測の事故に備えて傷害保険に加入しています。万が一事故が発生した場合の補償は、旅行業法に定められている特別補償規定の補償範囲内となります。/All participants are covered by accident insurance during the camp. In the unlikely event of an accident, any compensation will be limited to the scope of coverage provided under the accident insurance policy, in accordance with Japanese travel regulations.
② 活動中のリスクについて / Risks Associated with Camp Activities
本キャンプでは、自然環境の中で行うプログラムが多く、活動には一定の危険を伴うことを理解しています。主催者は、参加者の安全確保に最大限努めますが、施設・設備・備品・活動において、すべての危険や事故を完全に防ぐことを保証するものではありません。また、参加者が第三者に損害を与えた場合は、主催者の加入する保険の範囲内で補償いたします。一方、第三者から参加者に対して生じた損害については、主催者に補償を請求できないことを理解し、同意します。/ I understand that many camp activities take place outdoors and involve inherent risks. While KOA Summer Camp takes reasonable precautions to minimize these risks, it cannot guarantee that all facilities, equipment, or activities will be free from hazards or accidents. If a participant causes damage to a third party, compensation will be provided within the limits of the organizer's liability insurance. I also understand and agree that KOA Summer Camp cannot be held responsible for damage or loss caused to the participant by a third party.
③ 緊急時の対応について / Emergency Medical Authorization
緊急時には、KOA Summer Campが以下の手順で対応することに同意します。/ In the event of an emergency, I authorize KOA Summer Camp to follow the procedures below:

保護者へ連絡し、状況を報告します。/ Contact the parent or guardian and explain the situation.

保護者と連絡が取れた場合は、その指示に従います。/ If the parent or guardian can be reached, follow their instructions.

保護者と連絡が取れず、速やかな医療機関の受診が必要と判断した場合は、主催者の判断で医療機関へ搬送し、保護者への連絡を継続します。病院到着後は医師の指示に従い、必要な検査・投薬・治療を受けることに同意します。
※体調不良等により、保護者に宿泊施設までお迎えをお願いする場合があります。/ If the parent or guardian cannot be reached and immediate medical attention is deemed necessary, KOA Summer Camp may arrange transportation to a medical facility and continue attempting to contact the parent or guardian. Upon arrival at the medical facility, I authorize medical staff to provide any necessary examination, medication, or treatment as directed by the attending physician. If a participant becomes ill, a parent or guardian may be asked to pick up their child from the accommodation.
④ プログラムの変更について / Program Changes
台風その他の悪天候や、やむを得ない事情により、プログラム内容や日程を変更する場合があります。あらかじめご了承ください。/ Programs and schedules may be changed due to typhoons, severe weather, or other unavoidable circumstances. Thank you for your understanding.
